Central Utah Counseling Center is accepting proposals for architectural services associated with the following project:

Administration/Clinical Office – Central Utah Counseling Center

Ephraim, Utah

This project, when completed, will consist of the design and construction of a 5,500 to 6,500 square feet facility located at 160 N. 400 W. Ephraim, UT 84627.  Proposals for architectural services will be accepted until 2:00 PM on Thursday November 19th, 2009.  Bids will be received and the bid opening will be at Central Utah Counseling Center’s Business Office located at 255 W. The scope of work includes the following:

• Architectural Design of building
• Civil Engineering
• Landscaping design
• Structural Engineering
• Electrical Engineering
• HVAC Engineering (VAV System)
• City Zoning and Permits
• Parking Lot Design
• Field Supervision (Project Manager)
• Contractor Selection

Architect will be chosen based on the following criteria:

• Licensed Architect.
• Cost.
• Previous work experience with Central Utah Counseling Center.
• The experience of the firm with Mental Health/Substance Abuse facility design.
• The experience of the firm with Healthcare Facility design.
• The experience of the firm with Commercial Building design.
• Adequate Liability Insurance Coverage.
• References

CUCC administration will evaluate proposals based on a calculation of the above listed eight criteria and make a recommendation to the CUCC Board of Directors for approval.
